Was your business under partial or full suspension due to governmental orders due to COVID-19? Did you pay any employees during that time? 2. Were any employees absent from work, and paid sick leave or regular wages, due to the following reasons: diagnosed with COVID-19, primary care-taker of COVID-19 patient, or unable to work because of a need to care for a child whose school or place of care was closed due to COVID-19?
